My Sallet liners are sewn onto a leather strap that is riveted to the helm just like the link Wolf posted to myarmory. I also have an SCA helmet done just like RauttSkegg has done, the liner is sewn to the helmet via small holes drilled around the opening.

Making the liner was not as hard as you might think. I got the shape by pushing paper into the helmet and dividing it into quarters to get the top shaping. I sewed two layers of  linen together, it is stuffed with raw cotton (can get cotton batting at a Jo Annes or any other cloth shop) and I quilted it with a sailors needle and pushed it through using a board in my lap to if the cotton was hard to push the needle through with.
