Gentlemen:
Are any of you aware of where I could acquire a great helm in the style of the Maciejowski Bible? They seem a little different in design than the perfectly cylindrical great helms offered by most suppliers. Also, is there a recommended supplier of accurate mid to late 13th century heater shields with historically accurate straps/handles?

Bugger. I used to know where to get one of those helms but I can totally put out the word on an SCA FB group and the Armour Archive if you like.
Shields wise I'd say make it yourself. That way you get exactly what you want and learn a new skill. :)

i always have such a problem with the top when making one of these. i can never get the rim to bend down nice and even.

It took some number of months to get, since I sent them measurements for custom sizing, and ordered it with the leather suspension. It fits perfectly.

So, By The Sword for a good Maj barrel helm; go here for your heater: http://www.oakheartarmory.com - I use them almost exclusively, they're the cheapest you'll find that still has quality and they can set it up however you like: lefty, righty, guige strap or not, any heraldry design and color as well as rawhide, hose or steel edging (go for rawhide, more historical look), and of course, the angle of the strapping. Generally they put a curve to their shields unless you request otherwise.

Is by the sword connected to dark knight and medieval collectibles? Their website has always looked almost identical to me.

By the sword I believe is the original website. Others maybe copy cats or similar companies

They are different companies.

Yeah those guys also go by Dark Knight Armoury in an attempt to look like they're different companies. They are not a good quality company for equipment, but good for ren faire costumers and people who just want display gear. Their shipping times are pretty terrible and the equipment they sell is certainly not battle ready, though. I've wasted a lot of money with them back when I didn't know better.
